Philips Demonstrates New Telehealth Platform to Enable At-Home Care Programs at the National Association of Home Care and Hospice

Philips Hospital to Home will support nine customer presentations around best practices for at-home care, and provide demonstrations of eCareCoordinator and eCareCompanion

ANDOVER, Mass., Oct. 17, 2014 /PRNewswire/ -- Royal Philips (NYSE: PHG AEX: PHIA) today announced its participation at the National Association of Home Care and Hospice annual conference (NAHC) from October 19 – 21 in Phoenix, Ariz. Demonstrating its commitment to improving care outside the hospital walls, Philips Hospital to Home will highlight innovative technologies, including its two new FDA-cleared home telehealth applications, eCareCoordinator and eCareCompanion.

Part of Philips Hospital to Home's Transition to Ambulatory Care (eTrAC) program, which aims to reduce readmissions and lower costs, these two applications give patients and their care teams real-time insights into patient vital signs, trends, and other critical metrics.

eCareCoordinator provides clinicians with a daily review of each of their patients, allowing them to prioritize patients and adjust care plans or intervene as needed. The eCareCompanion application serves as the patient portal, driving patient engagement and self-management. The personalized application is accessed via a tablet, and allows patients to answer questions about their health and enter requested measurements, like weight and blood pressure, to stay connected with their care teams.

The Philips Hospital to Home booth (#525) will also spotlight technology from Philips Lifeline, including Lifeline with AutoAlert– its fall-detection technology – and medication dispenser.
